When you report a claim, please have the following information available to provide to the Claims Representative. This checklist is provided for your convenience in gathering the needed information for reporting your loss:

  1. Policyholder Name and policy number.
  2. Date, location, time, and facts about the accident or incident.
  3. If an accident, the names of all those who were involved.
  4. Police accident report (or its number), if available.
